Denial Management
Denial management is essential to maximizing the revenue cycle and ensuring timely reimbursement for medical services. Denial can occur due to various reasons such as patients' ineligibility, incorrect CPT codes, missing modifiers, invalid information, and medical records. Billing Care Assistant offers outstanding services to help you expand your business. By establishing reliable systems and services tailored to meet your demands, we will assist you in reducing the number of denied claims and missed reimbursements. Our focus is entirely on the denial prevention and improving the accuracy. Many denials start earlier in the cycle, so we pair this with clean claim submission, accounts receivable management and regular medical billing audits. Not sure where your revenue is slipping? How Our Denial Management Works
Every denied claim is money your practice has already earned. Our team works each denial from start to finish: we read the remittance (ERA or EOB), sort the denial by its CARC and RARC codes, and fix the cause, whether it is a coding error, a missing modifier, an eligibility problem or missing documentation. Then we send a corrected claim or file an appeal before the payer's deadline.
We track every claim until it is paid. Appeals that need a written letter are backed by the medical record, and claims that only need a correction are resubmitted as soon as the fix is made. Each month you see how many claims were denied, why, how much was recovered and what is still open, so nothing is quietly written off.
Preventing Denials at the Source
Fixing denials is only half the job. We look for patterns, such as the same payer, CPT code, modifier or missing authorization, and fix the step that causes them: insurance verification before the visit, prior authorization checks, coding review and claim scrubbing before submission. Common causes include CO-16 (missing information), CO-197 (missing authorization) and eligibility denials.
Because we also handle medical billing and coding, denial prevention is built into every claim we send, not added afterwards.

Denial Management FAQs
What is denial management in medical billing?
Denial management means finding out why an insurance plan refused a claim, correcting it and resubmitting or appealing it, and then fixing the cause so the same denial does not happen again. It recovers money now and prevents future losses.
What are the most common reasons claims are denied?
The most common causes are wrong or missing patient and insurance details, no prior authorization, services the plan does not cover or considers not medically necessary, coding and modifier errors, duplicate claims and claims filed after the deadline. Most can be caught before the claim is sent.
What is the difference between a rejected claim and a denied claim?
A rejected claim never entered the payer’s system because of an error such as a wrong member ID. It can be corrected and sent again. A denied claim was processed and refused, so it needs a corrected claim or a formal appeal.
How long do you have to appeal a denied claim?
It depends on the payer. Medicare allows 120 days from the initial decision to ask for a redetermination. Commercial plans often allow 60 to 180 days. We track each deadline so appeals are filed on time.
What is a good claim denial rate?
Many practices aim to keep first-time denials under 5 to 10 percent of claims. A higher rate usually points to a fixable problem at the front desk, such as insurance checks or prior authorizations.
