Common Lab Billing Problems We Fix
| Billing problem | Why it costs you | How we fix it |
|---|---|---|
| Missing or invalid diagnosis codes | Lab tests need a diagnosis that supports medical necessity | We check diagnoses against coverage policies before claims go out |
| Medicare frequency limits | Many tests, such as lipid panels, are limited by Medicare coverage policies | We flag tests that need an ABN before they are run |
| Panel and bundling errors | Billing single tests when a panel was run can lead to denials | We code panels and single tests correctly |
| CLIA number and certificate | Claims need the right CLIA number, and waived tests need the QW modifier | We include CLIA details on every claim |
| Incomplete requisitions | Missing patient or insurance details stop claims from going out | We follow up with ordering providers to complete the data |
Lab Billing FAQs
Why do lab claims get denied for medical necessity?
Payers, including Medicare through national coverage determinations, cover many tests only for specific diagnoses. If the ordering provider’s diagnosis does not support the test, the claim is denied.
When should a lab use an Advance Beneficiary Notice?
When a Medicare patient’s test may not be covered, for example because of frequency limits or the diagnosis, the patient should sign an ABN before the specimen is collected.
What is the QW modifier?
It is used on Medicare claims for CLIA-waived tests performed by labs with a Certificate of Waiver.
Do you bill both clinical labs and pathology?
Yes. We bill clinical laboratory testing and anatomic pathology. See our pathology billing page.
